Financial Policy

Our fees are based on the nature of treatment provided. Payments are expected at the time of the visit. We accept Visa and MasterCard.

We accept all PPO insurance as long as you can choose your provider. Dr. Bamini Blau is in-network with Cigna, Humana, Principle, Ameritus, and Unicare. Even if you have insurance, deductibles and co-pays are due at the time of the service. Please call 24 hours ahead of your child's appointment so we can verify your insurance and better assist you with your claims. Please remember that your insurance policy is a contract between you and your insurance carrier and you are responsible for any procedures that are not covered under your plan.
